Who is this for? This form is for pensioners of the Plan who wish to change their bank details. If the form is received after payroll cutoff (normally around 5th of the month) the change will be actioned on the following month’s payroll. If you are changing your bank details to an overseas account, you will need to upload a copy of a recent bank statement, that shows your name, address, and new account details. This is required for us to confirm your identity.
